Death Registration Appointment

Islington and the City of London Register Office offer an appointment service only.  An appointment can be made by completing this form.

Our opening hours are Monday-Friday 09:00-16:30.

Before you book your appointment please confirm that the following applies to you:

  1. I have read the information on who can register a death and I meet the specified criteria.
  2. I wish to register a death which occurred in Islington or the City of London.                                                                                                                                         OR                                                                                                                                                                                                                                                        I wish to give a declaration of death which occurred elsewhere. I understand that Islington Register Office is not able to register the death or issue any paperwork. I will bring payment for any certificates I require in the form of a cheque or postal order payable to the district where the death took place.                                                        
  3. I have a medical certificate confirming the cause of death.                                                                                                                                                                      OR                                                                                                                                                                                                                                                          The death has been referred to the Coroner and I have been informed that the necessary paperwork has been sent to Islington Register Office.
